This release is intended to discharge Clayat, its owners, officers, employees, contractors, directors, agents and successors, in advance, from any and all liability, except for their sole negligence or intentional acts, related to our participation in Clayat activities.

The activities offered by Clayat may be hazardous, strenuous, and/or physical and may result in injury, death or property damage.

California law requires a warning about long exposure to chemicals, including crystalline silica, which is known to cause cancer or birth defects or other reproductive harm. This warning can be found at www.p65warnings.ca.gov.

Despite the risk involved, I voluntarily request permission for myself or my minor children to participate in Clayat activities.

I assume any and all risks of injury, death, or property damage and agree to release and hold harmless Clayat, its officers, employees, contractors & agents, except for their sole negligence or intentional acts.

This waiver, release and assumption of risk is binding on me and my heirs and assigns.

I will indemnify and hold Clayat harmless from any loss, liability, damage, cost, or expense, including litigation, arising from any injury or property damage sustained during our participation in Clayat activities.

I will compensate Clayat for any loss, damage, or cost incurred due to any litigation arising from claims made by myself, my minor children, or anyone on their behalf.

Clayat's activities are not considered child care under California law.
